Service Agreement

Frontida Caring Services (ABN 73 695 471 802) — 22A Ferndale Rd, Revesby NSW 2212

1. Parties

This Service Agreement is between the participant and Frontida Caring Services (ABN 73 695 471 802) of 22A Ferndale Rd, Revesby NSW 2212. It commences on the agreed start date and continues until terminated per the conditions below.

2. Purpose

This agreement documents the supports provided by Frontida Caring Services, aiming to support independence, social and economic participation, and enable choice and control in the pursuit of goals.

3. Supports & Services

We will provide services as agreed upon. Together with you and your representative, we'll determine specific services through consultations, intake forms, and support planning. Services, times, locations, and staff will be agreed in writing and can be adapted as your needs change.

4. Consent

Your informed consent is required for all services. You may withdraw consent for any specific service at any time, and it will cease immediately.

5. Our Responsibilities

Frontida Caring Services agrees to:

  • Provide all supports in a timely manner that meets your needs.
  • Review supports at least annually.
  • Provide supports at your preferred times once agreed.
  • Communicate clearly, openly and honestly.
  • Treat participants and representatives with courtesy and respect.
  • Consult on decisions about how supports are provided.
  • Provide information about managing complaints.
  • Give minimum 24 hours notice for schedule changes.
  • Protect your privacy and confidential information.
  • Keep accurate records on supports provided.

6. Participant Responsibilities

  • Inform us how you wish services to be delivered.
  • Collaborate in developing and reviewing your plan.
  • Provide accurate, up-to-date information.
  • Communicate openly and honestly.
  • Treat all staff with courtesy and respect.
  • Give required notice for missed appointments.
  • Pay invoices for agreed services promptly.

7. Fees & Payments

We charge for services after delivery. Prices are subject to applicable pricing arrangements. Additional unfunded expenses are the participant's responsibility.

8. Cancellation Policy

We may charge 100% of the fee if you do not show up within a reasonable time or give less than seven days' cancellation notice. To cancel outside office hours, call 0431 162 916.

9. Termination

Either party may end this Agreement with 28 days' written or verbal notice. Immediate termination applies for serious breaches, cessation of funding, extended no-contact (2 months), or unacceptable behaviour.

11. Emergency Management

In emergencies, we follow our care continuity plan: finding a qualified substitute worker, engaging external agencies if needed, or recruiting replacements. You will be kept fully informed throughout.
